病理
iNPHの剖検例1）-16）や生検組織の病理についていくつかの報告があるが，現在でもiNPHを特徴づける病理所見は明らかになっていない。全脳が検索された病理所見として，脳軟膜・くも膜の線維化・肥厚，くも膜顆粒の炎症性変化，脳室壁の上衣細胞の脱落，上衣下のグリオーシス，くも膜下腔や脳実質の血管壁の硬化性変化や脳実質の虚血性病変，Alzheimer病の病理変化（老人斑や神経原線維変化）などが記載されるが，症例ごとにさまざまである1）-16）。iNPHは高齢者に好発するため，老人斑，神経原線維変化や虚血性病変が混在する例が多いと考えられる。
一方，本邦の学会発表などでiNPHの病理学的基盤として進行性核上性麻痺（progressive supranuclear palsy；PSP）などの神経変性疾患が議論されている。それらの議論の発端となった研究では，PSPなどの神経変性疾患の病初期にはiNPHとの鑑別が難しい臨床像を呈することがあるとして，注意を喚起している。それらの症例も注意深く経過観察を行うと，病期の進行に伴いPSPに特徴的な神経徴候が出現し，最終的には神経系の専門医によりPSPと臨床診断されている17）18）。
