【背景・目的】
近年，乳癌の手術術式は大きく変化した。すなわち，ハルステッド術に代表される胸筋合併乳房切除術は胸筋温存乳房切除術にその主座を譲り，さらに乳房温存術が早期乳癌の標準的治療法の一つとしてその地位が確立された。乳房温存術の施行頻度は年々増加し，2000年の時点で全術式の40.8％に施行されている。ここでは，乳房温存術と乳房切除術の間で術後のQOLに差があるか否かを評価する。
【解説】
乳房切除術，乳房温存術の間でQOLを比較した1966年以降のメタアナリシス，ランダム化比較試験を対象とし，1件のシステマティック・レビューとこれに含まれない8件のランダム化試験を選択した1）,2）,3）,4）,5）,6）,7）,8）,9）。ここで扱われているQOLは身体面，精神/心理面，社会面などの健康関連QOLと性，身体イメージ，再発の恐れなどであるが，結果がほぼ一致しているのは身体イメージのみで，乳房温存術のほうが乳房切除術に比べ勝っている。いずれの研究でもQOLは二次エンドポイントとして評価されており，使用した調査票，評価項目，調査時期などが一貫していないこと，術後短期での比較が多く長期間での比較はなされていないなどの問題点もあるが，必ずしも健康関連QOLおよび性，再発の恐れなどの面で乳房温存術が勝っているとはいえない。
