6-10-3.副鼻腔炎の治療と気管支喘息

気管支喘息と慢性副鼻腔炎の合併は以前より指摘されており,喘息患者においては40-60%がX線上副鼻腔に異常所見を示すともいわれる。一般には慢性副鼻腔炎が喘息に先行する場合が多いが,同時発症のこともある。

科学的根拠

副鼻腔炎の治療が気管支喘息症例の症状や呼吸機能に与える影響を検討した論文が6編ある。その内1編は副鼻腔手術の有無によって解析,評価した群間比較試験であって,術後6ヶ月での有意なピークフロー値の改善が報告されている¹⁾。また,副鼻腔手術施行例が扁桃切除施行例に比してより高い症状改善率を示したとの報告²⁾も認められる。その他,非比較試験であるが外科的処置^3),4),あるいは抗菌薬^4),5) による副鼻腔炎治療にて,少なくとも一部の症例で喘息症状が軽減し呼吸機能が改善したと報告されている。だがこれらの報告はいずれも少数例での時系列研究を中心とした検討であり,また手術によっても喘息症状は改善しないという報告⁶⁾もあって,根拠に基づいた結論を得るには多数例での群間比較試験が必要であろう。
